When you use the contact or booking form we collect your name, email address, telephone number where you give it, and anything you choose to write in the message field.

We ask you not to include medical details in the website form. Clinical information is collected separately, once your assessment is arranged, through our clinical records system.

We use your details to reply to your enquiry, arrange your assessment, and keep a record of the care we provide. We rely on legitimate interests for responding to enquiries and, where you become a patient, on the legal bases that apply to the provision of health care.

We do not sell your information and we do not use it for advertising.

General enquiries that do not lead to an appointment are deleted within twelve months. Clinical records are kept for the period required of registered optometrists in the United Kingdom.

We share information only with the suppliers who help us run the practice, such as our website host, email provider and clinical records system, and only so far as they need it to provide that service. Where required, we may share information with other health professionals involved in your care.

You can ask for a copy of the information we hold about you, ask us to correct it, ask us to delete it where we are not required to keep it, and object to certain uses. Contact us and we will respond within one month.

If you are unhappy with how we have handled your information you can complain to the Information Commissioner's Office at ico.org.uk.
